Job summary

Choice Hospice, Inc. is seeking a Nurse Practitioner to provide hospice care in Tucson, AZ. The role involves evaluating and managing terminal illness in collaboration with the IDG, educating patients and families, and conducting face-to-face visits at the patient’s residence.

The successful candidate will perform clinical assessments, coordinate with hospice physicians, and stay current on pharmacology and palliative medicine knowledge in a dynamic team environment.

Qualifications

• Current STATE RN license.
• Advanced Practice Nurse license.

Responsibilities

• Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
• Perform Face-to-Face visits at the patient's place of residence.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
• Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
• Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
• Knowledgeable of hospice criteria.

The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate- a proven clinician responsible for helping shepherd the care of the seriously ill.
