Job summary

Amedisys, part of the Optum family, is seeking a Per Diem Hospice Nurse Practitioner in New Jersey. You will provide patient-centered end-of-life care, focusing on comfort, dignity, and quality of life, working with an interdisciplinary hospice team to assess needs and manage symptoms.

Responsibilities include communicating findings, conducting face-to-face evaluations, ordering diagnostics, making referrals, and participating in team conferences.

Qualifications

• Current and unencumbered NP license in the state of assignment.
• 1+ year NP experience or 1 year hospice RN in last 3 years.
• Authority to practice and to prescribe as an NP in the state of service.
• Medicare billing privileges as an NP.
• State-specific collaborative requirements and ability to transfer agreements if applicable.
• State-mandated insurance to practice and prescribe.

Responsibilities

• Communicate significant findings and changes to the interdisciplinary team.
• Complete face-to-face encounters and assess disease progression and symptom control.
• Order/review labs, imaging, and diagnostics for palliation and document plans with physician.
• Make referrals to other disciplines as indicated by patient needs.
• Participate actively in interdisciplinary team conferences.
