Job summary

Agape Hospice NW in Oregon seeks a Nurse Practitioner (NP) to provide comprehensive hospice clinical care in collaboration with the interdisciplinary team. You will evaluate patient needs, develop Plans of Care, provide clinical oversight, and support high-quality hospice services.

The role emphasizes comfort, symptom management, and continuity of care with physicians and nurses. The NP will perform visits, manage symptoms, and participate in IDT meetings.

Qualifications

• Must hold an Oregon NP license and board certification.
• DEA registration and prescriptive authority as applicable.
• Hospice/palliative care experience preferred.

Responsibilities

• Conduct comprehensive patient assessments to identify hospice needs.
• Manage symptoms, pain, and comfort to improve quality of life.
• Develop and update Plans of Care with the attending physician.
• Perform hospice visits including routine, urgent, and follow-up.
• Collaborate with the medical director, physicians, and team on care plans.
• Provide education and support to patients and families on disease progression and treatment.
• Document care in accordance with CoP, state and federal requirements.
