Job summary

Agape Care Group is seeking a Hospice and Palliative Care Nurse Practitioner (PRN) to deliver compassionate care to seriously ill patients and alleviate burdensome symptoms. You will work with a team to align treatments with patients' goals and values, and provide hospice services and education in patients' homes.

You will join a supportive leadership team, with opportunities for professional development and advancement within our regional hospice network.

Qualifications

• Graduate of an accredited nursing program (Certificate/Diploma/Associate/Bachelor) preferred.
• 2 years experience in chronic disease management/long-term care; Hospice background preferred.
• Reliable transportation; ability to sit, stand, bend, move intermittently and lift 80–100 lbs effectively.

Responsibilities

• Provide hospice and palliative care to seriously ill patients to alleviate symptoms.
• Collaborate with clinicians to align treatments with patients' goals and values.
• Deliver hospice service and education in patients' homes and community settings.
