Job summary

Agape Care Group seeks a Hospice Nurse Practitioner to provide compassionate, patient-centered hospice care to seriously ill patients in home-based settings and through our care teams.

You will work with clinicians to alleviate symptoms, coordinate care with families, and educate families about hospice services and expectations. Our team offers supportive leadership, professional development, and opportunities for advancement.

Qualifications

• Graduate of an accredited Nursing program (certificate/diploma/associate or bachelor).
• 2 years experience in chronic disease management/long-term care within last 5 years; hospice background preferred.
• Heart to serve patients and families; passion for providing the best possible care.
• Reliable transportation; ability to sit, stand, bend, move intermittently and lift 80–100 lbs.

Responsibilities

• Cares for seriously ill patients to alleviate symptoms and align treatments with goals.
• Collaborates with a team of clinicians to deliver quality hospice care.
• Provides hospice service and education to patients and families across settings.
