Job summary

Agape Care Group is seeking a Hospice Nurse Practitioner to provide compassionate care for seriously ill patients, align treatments with patients' goals, and alleviate burdensome symptoms. You will collaborate with a team of clinicians to deliver high-quality care in patients' homes and communities.

You will join a supportive leadership group and have access to professional development and advancement opportunities within our hospice and palliative care family.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
