Agape Care Group in Virginia is seeking a Hospice and Palliative Care Nurse Practitioner to deliver symptom management and align treatments with patients' goals and values. You will join a team of clinicians to provide compassionate care wherever patients call home.

As an NP, you will contribute to ongoing palliative care and hospice education, with access to leadership support and professional development opportunities to advance in a caring culture.

Qualifications

• Heart to serve patients and families and a passion for providing the best possible care.
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred.
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.

Responsibilities

• Care for seriously ill patients to alleviate burdensome symptoms and align medical treatments with patients' goals and values.
• Collaborate with a team of clinicians to deliver hospice and palliative care and education in patients' homes.
• Provide ongoing palliative care and hospice service within the territory and participate in professional development.
