Job summary

AHN Healthcare@Home in New Kensington, PA, seeks a part-time Hospice Nurse Practitioner to provide end-of-life care in patients' homes. A $10,000 first-year bonus is available for milestones achieved during the first year.

Responsibilities include clinical assessments, symptom management, and interdisciplinary collaboration to enhance comfort and quality of life for patients and families. Requirements include BSN and MSN degrees, at least five years of clinical experience, and active RN/NP

Qualifications

• BSN from an accredited School of Nursing.
• MSN from an accredited School of Nursing.
• Minimum of five years of clinical experience.
• Licensed RN and NP in the state of work.
• Experience in Home Health or Hospice/Palliative Care preferred.

Responsibilities

• Provide compassionate, patient-centered hospice care in patients' homes.
• Conduct comprehensive assessments and manage complex symptoms.
• Develop individualized care plans aligned with patient goals.
• Educate families and caregivers and support decision-making.
• Maintain accurate EMR documentation and work independently while collaborating with the team.

What You’ll Do:

• Provide compassionate, patient-centered hospice care, collaborating with an interdisciplinary team to enhance comfort, dignity, and quality of life for patients and families
• Conduct comprehensive clinical assessments, manage complex symptoms and medications, and develop individualized plans of care that align with patient goals and end-of-life preferences
• Educate and support patients, families, and caregivers through clinical decision-making and care planning while ensuring accurate, timely, and compliant EMR documentation through strong critical thinking and problem-solving skills
• Work independently, but never alone
• Travel to patient homes within assigned territory

Nurse Practitioner Qualifications

• Bachelor of Science (BSN) degree from an accredited School of Nursing
• Master of Science (MSN) degree from an accredited School of Nursing
• A minimum of five (5) years of clinical experience
• Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work
• Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred
