Job summary

AnchorHealth Hospice is seeking a qualified Nurse Practitioner to provide comprehensive hospice services under the supervision of a collaborating physician in California. You will evaluate health history, manage care plans, prescribe medications, and coordinate with patients and families in home-based settings.

The role requires a Master's degree in Nursing, NP certification, CA RN license, and at least one year of NP experience with hospice or palliative care.

Qualifications

• Graduation from an accredited nursing program and current RN license.
• Certified Nurse Practitioner with California NP certification.
• Master's degree in Nursing with minimum 1 year NP experience.
• Experience in hospice or palliative care preferred; CHPN certification bonus.

Responsibilities

• Provide medical care and palliate terminal illness under physician supervision.
• Complete history and examinations, interpret tests, document findings.
• Develop individualized care plans and prescribe medications as needed.
• Coordinate hospice services with the interdisciplinary team.
• Consult the physician for plans outside standard practice when required.
• Educate patients and families, ensure safety and continuity of care.
