Job summary

Ennoble Care is seeking an experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients in the Topeka, Kansas region on a part-time, pay-per-visit basis. You will apply medical techniques under physician supervision and care for patients with chronic diseases.

Qualifications

• Board Certified NP or PA with valid Kansas license.
• 1–2 years of primary care experience with geriatric patients.
• Hospice experience preferred but not required; ability to work independently.

Responsibilities

• Provide high-quality primary and palliative care to homebound patients in and around Topeka, KS.
• Assess health through physical examinations and medical histories; craft care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Manage a panel of patients with patient-centered care.
