Job summary

CommuniCare Family of Companies’ Dignity Care Hospice in St Clairsville, OH is seeking a Hospice Nurse Practitioner to provide advanced nursing care, educate families, and coordinate with the Medical Director to optimize end-of-life care at patients’ homes.

You will perform patient visits, assess eligibility, maintain meticulous records, and ensure compliance with evolving federal, state, and accreditation standards while supporting the interdisciplinary team and participating in ongoing

Qualifications

• Valid Nurse Practitioner license.
• Master’s degree in nursing.
• Previous hospice and palliative care experience preferred.
• Must have reliable transportation.

Responsibilities

• Perform face-to-face encounters with the patient to determine continued eligibility for hospice care within regulatory guidelines.
• Document clinical findings in detail at patient bedside and provide a clinical report to the hospice physician within 24 hours of visit.
• Prepare and maintain accurate patient records and documentation to support interdisciplinary team and services provided.
• Comply with legal, regulatory, and accreditation requirements.
• Evaluate hospice patients for appropriate level of care.
• Assist in development of hospice protocols, policies, and procedures as required.
• Participate in QAPI activities as requested.
• Participate in in-service education related to areas of practice as required.
• Address and support cultural practices, as long as practices do not harm others.
• Adhere to organization policies and procedures.
• Maintain knowledge of current federal, state, and accreditation standards and organization regulations concerning patient care.
• Complete annual educational requirements.
• Ensure compliance with state and federal regulations, company policies/procedures, and process maps in the performance of job responsibilities.
• May provide on call support to staff.
• Contribute to team effort by accomplishing related results as needed.
