Remarkable Hospice is seeking a Hospice Nurse Practitioner to join our interdisciplinary team. You will assess patients, develop and implement care plans, manage symptoms, and support families through the end-of-life process.

The role emphasizes collaboration with physicians, nurses, social workers, and chaplains, with emphasis on documentation, quality improvement, and compassionate, patient-centered care across diverse settings.

Qualifications

• MSN or DNP required.
• 3 years hospice/palliative experience preferred.
• Driver’s license and auto liability insurance; licensed to practice in the state employed.

Responsibilities

• Assess patients’ physical, emotional, and psychosocial needs.
• Develop, implement, and review individualized care plans with the interdisciplinary team.
• Manage pain and other symptoms; prescribe treatments as appropriate.
• Educate patients and families on disease processes, symptom management, and end-of-life decisions.
• Collaborate with physicians, nurses, social workers, chaplains, and other team members to coordinate care.
• Maintain accurate documentation of assessments, plans, and treatments in line with policies.
• Participate in quality assurance and performance improvement initiatives.
• Assist during patient crises and facilitate transitions to higher levels of care.
• Mentor and educate nursing staff; engage in continuing education.
