CommuniCare Family of Companies seeks a Hospice Nurse Practitioner to provide patient care and families education through advanced nursing practice. You will collaborate with the Hospice Medical Director to promote health, manage chronic conditions, and deliver end-of-life care through home visits in the Akron area.
The role requires a valid NP license, a Master’s in nursing, and hospice experience preferred.
