CommuniCare Family of Companies is seeking a Hospice Nurse Practitioner to join Dignity Care Hospice in Indianapolis. You will provide advanced nursing care to patients at home, collaborate with the Hospice Medical Director, and educate families on end-of-life care.
This role supports health promotion and chronic disease management while delivering compassionate service. The position requires a valid NP license, a master's in nursing, and reliable transportation.
