Job summary

Dignity Care Hospice in Ohio is seeking a Hospice Nurse Practitioner to provide high-quality care to patients at home and educate families as part of an interdisciplinary team.

You will work with the Hospice Medical Director to manage health promotion, chronic disease management, and end-of-life care, performing face-to-face visits to assess eligibility and maintaining accurate records in line with state and accreditation standards.

Qualifications

• Valid Nurse Practitioner license and Master’s degree in nursing.
• Experience in hospice/palliative care preferred.
• Must have reliable transportation and be able to travel to patient homes.

Responsibilities

• Perform face-to-face encounters with the patient to determine continued eligibility for hospice care.
• Document clinical findings in detail at patient bedside and provide a clinical report within 24 hours of visit.
• Maintain accurate patient records and documentation to support interdisciplinary team and services.
• Comply with legal, regulatory, and accreditation requirements.
• Evaluate hospice patients for appropriate level of care.
• Assist in development of hospice protocols, policies, and procedures as required.
• Participate in QAPI activities as requested.
• Adhere to all organization policies and procedures.
• May provide on-call support to staff.
