Hospice of Huntington, Inc. in Huntington, WV seeks a Nurse Practitioner to assess, treat and educate hospice patients in home settings and at Emogene Dolin Jones Hospice House. The NP will work with the medical director to manage pain and symptoms and lead staff training on end-of-life care.

The role emphasizes collaboration with the interdisciplinary team, patient advocacy, and ongoing quality improvement while maintaining confidentiality and cost-effective care delivery.

Formación

• Completion of Accredited Nurse Practitioner Program.
• Master's Degree in Nursing.
• Current license to practice as a registered nurse in Ohio and West Virginia.
• Current certification as family or adult nurse practitioner.
• Two years experience as Nurse Practitioner.

Responsabilidades

• Rerforms consultative visits for certification and recertification assessment.
• Performs consultative visits for pain and symptom management.
• Offers clinical changes to plan of care with collaboration from attending physician and/or medical director.
• Prepares staff and community training on clinical aspects and end-of-life care.
• Reviews diagnosis, assessment criteria, and pain/symptom management with medical directors.
• Participates in research activities and quality improvement processes.
• Maintains confidentiality of patient/family information.
• Delivers hospice information to patients and families.
• Initiates orders with consult of the attending medical director.
• Assesses health status and plans/implements care needs.
• Plans therapeutic regimens to minimize pain, consulting with the health care team.
• Performs history and physical examinations and documents data.
• Identifies abnormal findings and consults with physicians.
• Teaches care to patients, families, and staff.
• Provides emotional support to patients and families.
• Supports skilled nursing care and case management in home and hospice settings.
• Collaborates with interdisciplinary team for care planning.
• Documents care and responses; adheres to agency policy.
• Teaches hospice philosophy and pain management to staff.
• Participates in orientation of new team members and residents.
• Respects confidentiality and delivers cost-effective care.
• Contributes to policy development and service programs.
• Pursues professional growth and constructive feedback.
• Upholds the mission of Hospice of Huntington, Inc.
