Job summary

Adoration Health in Parkersburg, WV is seeking a dedicated Nurse Practitioner to join our Hospice team. You will deliver compassionate medical care, symptom management, and coordinated hospice services with an interdisciplinary team.

Qualifications

• Graduate of an accredited School of Nursing and Nurse Practitioner program.
• Master’s degree with NP certification and licensure.
• Current nursing license in the state of practice.
• Current CPR and DEA licenses as required by state regulations.
• Nurse Practitioner Certification in Gerontology, Adult, or Family preferred.

Responsibilities

• Provide compassionate medical care and symptom management.
• Perform assessments, order/interpret diagnostic tests, and develop care plans.
• Prescribe medications, manage treatments, and coordinate hospice services.
• Conduct and document face-to-face visits for eligibility and recertification.
• Collaborate with physicians and IDT team to align care with patient goals.
• Participate in care planning and coordinate with team and external providers.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Educate patients & families on medical, emotional, and spiritual needs.
• Contribute to quality improvement efforts and professional development.
• Serve as a clinical resource and liaison to community partners
