Job summary

SouthernCare New Beacon Hospice seeks a Nurse Practitioner to deliver patient-centered hospice and palliative care across Marengo, Sumter, Greene, and Hale counties. The role includes patient visits, eligibility assessments, and care coordination with the IDG/IDT to manage complex symptoms and plans.

The successful candidate will perform CMS F2F visits, collaborate with physicians, and support families through end-of-life decisions.

Qualifications

• Master of Science in Nursing (MSN) or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Complete all visit types for hospice patients, including CMS-required hospice F2F visits within timeframes.
• Collaborate with Hospice and Palliative Medical Directors and interdisciplinary teams.
• Coordinate patient Plan of Care with PCP/Attending Provider and specialists.
• Perform in-depth patient assessments including physical, emotional, functional, and psychosocial evaluations.
• Support individualized care plans with patients, families, and IDG/IDT.
• Prescribe medications and treatments as permitted by state law and practice agreement.
• Order and interpret laboratory and other diagnostic tests within scope.
• Facilitate goals-of-care discussions and advance care planning.
