Job summary

Heart to Heart Hospice is seeking a Nurse Practitioner to provide compassionate, patient-centered care in a hospice setting in Kentucky. The APN will lead clinical assessments, educate patients and families on disease processes and end-of-life care, and collaborate with the IDG to develop quality care plans.

The role emphasizes timely documentation, coordination with physicians, and adherence to regulatory requirements while supporting patients and their loved ones during life-limiting illnesses.

Qualifications

• Graduate of a Baccalaureate School of Nursing.
• Current RN and APN licenses.
• Two years APN experience in clinical care, hospice or home health preferred.

Responsibilities

• Educate patients and families about disease process, self-care and end-of-life care.
• Assist in IDG development of the patient/family plan of care to assure quality care.
• Collaborate with attending and consulting physicians on palliative care needs.
• Inform physicians and staff of changes in patient needs and response to care.
• Submit all documentation in a timely manner.
• Perform other duties and ensure compliance with laws and policies.
