Qualifications

• Graduate of a Baccalaureate School of Nursing.
• Current State license as RN and as APN.
• Driver's license with reliable transportation.
• Two years of APN experience; hospice/home health preferred.
• Knowledge of pharmacology, palliative care, and end-of-life care.

Responsibilities

• Consult with patients and families about disease, self-care, and end-of-life care.
• Assist in IDG's care plan development to ensure quality care.
• Collaborate with physicians regarding palliative care needs.
• Inform on changes in patient needs and outcomes of care.
• Submit documentation in a timely manner.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and policies.
