Job summary

Heart to Heart Hospice is seeking a Nurse Practitioner to provide compassionate palliative care in a home-based setting in Indiana. You will educate patients and families about disease processes and end-of-life options, and collaborate with the IDG to develop care plans.

Ideal candidates hold an NP/APN license, a nursing degree, and a valid driver's license with reliable transportation. Hospice experience preferred, and you will ensure timely documentation and compliance with regulations.

Qualifications

Graduate of a Baccalaureate School of Nursing.
Current State license as a Registered Nurse and as an Advanced Practice Nurse (APN).
Current Driver's License, reliable transportation, and ongoing auto liability insurance.
Two years of experience as an APN in a clinical care setting, hospice or home health preferred.
Comprehensive knowledge of pharmacology, palliative care, and end-of-life.

Responsibilities

Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care.
Consult with and assist in the IDG’s development of patient/family plan of care to assure quality care is provided.
Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
Inform physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.
Timely submission of all documentation.
Performance of other duties as required.
Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
