Job summary

Providence at Home with Compassus is seeking a compassionate Nurse Practitioner to provide advanced nursing care across home-based hospice and palliative care settings. The NP will coordinate plans of care, perform end-of-life assessments, and collaborates with physicians and the care team under protocols.

The role includes on-call coverage, patient and family education, and ensuring regulatory requirements are met while maintaining consistent documentation and quality of care.

Qualifications

• Master's Degree in Nursing is required.
• 2 years of Acute or transitional care experience is required.

Responsibilities

• Actively conducts comprehensive medical, functional, and psychosocial assessments per protocol.
• Orders drugs and supplies under protocol; documents actions.
• Performs Face 2 Face, End of Life, and recertification assessment visits.
• Collaborates with the interdisciplinary team to coordinate and optimize patient care.
• Educates patients and families about hospice and palliative care options.
• Maintains timely and accurate documentation in the medical record.

Education
