Job summary

Heartland Hospice in Grand Rapids, MI is seeking an experienced Hospice RN / RN Case Manager to coordinate home-based care for patients with life-limiting illness.

You will lead an interdisciplinary team, conduct assessments, educate families, manage plans of care, and ensure compassionate end-of-life support with thorough documentation and compliance. This role emphasizes empathy and timely communication to improve quality of life for patients and their loved ones.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospice/home health or related field.
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary point of contact for patients and families.
• Deliver skilled nursing care per Plan of Care in home settings.
• Perform initial and ongoing patient assessments.
• Collaborate with IDG to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Teach caregivers practical training and reassurance.
• Communicate updates to care team for timely interventions.
• Maintain accurate clinical documentation and infection control.
• Support quality improvement and scheduling initiatives.
• Participate in on-call and patient support activities.
