Job summary

Horizon Hospice Care is seeking a Registered Nurse Case Manager in Michigan to deliver comprehensive nursing care to patients and families within the hospice program.

The role includes assessments, care planning, medication administration, and coordination with physicians and interdisciplinary team to ensure quality end-of-life care. Strong communication and organizational skills are essential.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Current valid driver’s license.
• At least one year of Hospice experience (preferred).
• Demonstrates excellent observation, problem solving, verbal and written communications, nursing skills per competency checklist.
• Shows ability to organize and prioritize workload independently.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient’s status and revising the plan of care as necessary.
• Develops a care plan based on nursing diagnosis and involves patient and family.
• Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing needs and self-care techniques.
• Maintains accurate clinical notes and reportsChanges in pain/symptom management.
• Coordinates patient care with physician and interdisciplinary team.
