Job summary

Affinity Health Management in Ohio is seeking a Registered Nurse to lead hospice care, planning and directing services in patients' homes and communities, drawing on community resources.

You will perform comprehensive assessments, develop and revise care plans, coordinate with physicians and the interdisciplinary team, provide patient and family education, and participate in on-call and quality improvement activities.

Qualifications

• Must be a Graduate of an accredited school of nursing.
• License to practice as a Registered Nurse in good standing.
• Minimum of 1 year of nursing experience within last 3 years, or a BSN with 2 years' experience.
• Management experience not required; may supervise hospice aides.
• Background checks required by regulation.
• Excellent observation, communication and problem-solving skills.
• Ability to lift, position and transfer patients; CPR certification required.

Responsibilities

• Conduct initial and ongoing patient/family assessments to determine hospice needs.
• Provide professional nursing care and implement the plan of care.
• Coordinate with physician and interdisciplinary team for patient care.
• Maintain accurate clinical notes and report changes in patient condition.
• Participate in on-call duties and quality improvement activities.
