Job summary
Compassionate Care Hospice, an Amedisys Company, is seeking a per diem Nurse Practitioner for hospice care in Sebring, FL. This role requires travel between facilities and offers a flexible schedule with competitive pay and benefits.
You will perform face-to-face visits, assess symptoms, manage medications, and collaborate with an interdisciplinary team to support quality end-of-life care.
Qualifications
- Nurse Practitioner license in the state(s) of work.
- Hospice NP experience and HCHB experience.
- Current CPR certification.
- Excellent communication and teamwork abilities.
- Willingness to travel daily between facilities.
Responsibilities
- Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and other members of the interdisciplinary team.
- Complete face to face encounters and assess disease-related changes, decline in function, adherence and symptom control, and quality of life.
- Order and review labs, imaging, and other diagnostics for palliation and treatment; document plans and review results with the collaborating physician.
- Make referrals to other disciplines as indicated by patient needs.
- Actively participate in interdisciplinary team conferences.
