Compassionate Care Hospice, an Amedisys Company, is seeking a Part-time RN Field Nurse for Hospice in Clermont, FL. The role requires travel within Lake County and features a schedule of 8:00 AM–8:00 PM on Friday & Saturday or Sunday & Monday.

Qualifications

• Current RN license in the state you will work.
• Minimum 1 year of strong clinical RN experience in an acute healthcare setting.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

**This position is based in Clermont, FL and requires travel within Lake County. Schedule: Friday & Saturday or Sunday & Monday, 8:00 AM - 8:00 PM.**

Salary Range:

$39- 42 hourly. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Required:

• Current RN license in the state(s) where you will work
• Minimum 1 year of strong clinical RN experience in an acute healthcare setting (Required)
• Current CPR certification

Preferred:

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
