Job summary

Compassionate Care Hospice in Sebring, FL is seeking a Nurse Practitioner – Hospice to deliver compassionate, patient-centered care. This full-time role requires travel between facilities for face-to-face visits and offers a flexible schedule with orientation, onboarding, and training.

You will work with an interdisciplinary team to assess symptoms, adjust pain management, order appropriate tests, and develop care plans.

Qualifications

• NP hospice experience is required.
• Current NP license in the state of practice.
• Ability to obtain Medicare billing privileges.
• CPR certification.
• Strong communication and interpersonal skills.
• Willingness to travel between facilities daily.

Responsibilities

• Communicate significant findings to the clinical manager and team.
• Complete face-to-face encounters and assess symptoms and QoL.
• Order and review labs and imaging as appropriate and document the treatment plan.
• Make referrals as indicated and participate in interdisciplinary conferences.
