Job summary

North Country Healthcare in New Hampshire seeks a Hospice Advanced Practice Registered Nurse (APRN) to deliver comprehensive palliative and hospice care to patients with terminal illnesses. Working with the Hospice Medical Director, attending physicians, and an interdisciplinary team, the APRN evaluates patients, manages symptoms, and develops and revises treatment plans.

This role promotes quality end-of-life care in line with CMS Hospice Conditions of Participation, while educating patients

Qualifications

• Master’s or Doctoral Degree in Nursing from an accredited program.
• Hospice certification (ACHPN or equivalent) preferred; BLS/ACLS/PALS as applicable.
• Licensed as an APRN in New Hampshire; DEA license; valid driver’s license.

Responsibilities

• Provide comprehensive hospice and palliative care to patients with terminal illnesses.
• Collaborate with the Hospice Medical Director, physicians, and the interdisciplinary team to evaluate patients and manage symptoms.
• Develop and revise individualized care plans; educate patients and families about disease progression and goals of care.
