Job summary

Community Hospice & Palliative Care is seeking an Advanced Practice Registered Nurse to join our nonprofit team delivering compassionate palliative and hospice care. The APRN will collaborate with physicians and the interdisciplinary team to provide expert evaluations, treatment, and management for patients with serious or terminal illnesses, in various settings including home and inpatient centers.

The role includes prescribing treatments, supporting goals of care discussions, educating staff

Qualifications

• Minimum two years of nursing experience.
• Master’s degree from an accredited Advanced Practice Nursing program.
• Current Florida Advanced Practice Nurse license.
• Valid Florida driver’s license.

Responsibilities

• Provide pain and symptom management for hospice patients.
• Act as attending provider in collaboration with hospice physicians.
• Coordinate admissions and discharges for hospice beds.
• Develop care plans and evaluate patient outcomes.
• Educate staff, patients, and families about palliative care.
• Maintain medical records and ensure informed consent documentation.
• Participate in on-call rotations with the interdisciplinary team.
