Qualifications

• Active RN and APRN license(s) in state(s) of practice.
• Active national board certification in area of clinical practice.
• Active DEA registration prior to hire.
• Illinois Controlled Substance License prior to hire (IL practice).
• BLS and/or ACLS, PALS, NRP as required.
• Education: Master's Degree in Nursing
• Experience: NP practicing experience required

Responsibilities

• Conduct comprehensive face-to-face hospice recertification visits.
• Perform head-to-toe assessments and evaluate disease progression.
• Review medical records, clinical documentation, and hospice plans of care.
• Complete timely documentation supporting eligibility and recertification.
• Identify changes in condition and decline during the previous benefit period.
• Collaborate with interdisciplinary hospice team.
• Participate in care planning discussions and clinical recommendations.
• Maintain compliance with regulatory documentation standards.
