Job summary

Lake Sunapee Region VNA & Hospice is seeking an experienced Nurse Practitioner to join our Hospice program in a per diem capacity. The role includes direct patient care visits, symptom management and Hospice Face to Face assessments, with on-call rotation.

We are a mission-driven provider delivering home health and hospice services throughout New Hampshire, seeking compassionate NPs to support patients and families in their homes and communities.

Qualifications

• Current NH Nurse Practitioner (NP) license.
• Minimum 2 years of healthcare experience.
• Hospice, palliative or end-of-life care experience preferred.

Responsibilities

• Provide direct patient care visits for routine provider visits and symptom management.
• Perform Hospice Face to Face assessments and participate in on-call rotation.
• Educate patients, families and facilities with empathetic guidance on end-of-life decisions.

• Experience in hospice and/or end of life care, and sympathetic management
• Strong communication with demonstrated ability to collaborate with an interdisciplinary team
• Comfortability with educating and guiding patients, families, facilities and community in end-of-life discussions, and decision-making
