Job summary

Interim HealthCare Inc. is seeking a Hospice and Palliative Care Nurse Practitioner (NP) to provide compassionate, patient-centered care across the care continuum.

The NP will perform bedside evaluations, collaborate with palliative care physicians, and manage treatment plans for acute and chronic conditions while ensuring proper documentation for reimbursement and quality care. This role emphasizes integrity and patient comfort.

Qualifications

• Current APN and RN license in the state of practice.
• Controlled Substance license required.
• Hospice/palliative care experience preferred.

Responsibilities

• Perform bedside palliative care evaluation and assist with diagnosis, treatment and management of acute and chronic conditions.
• Consult with palliative care physician or designees and inform primary physician of services provided.
• Prescribe medications including controlled substances as licensed.
• Maintain accurate patient records, charts and documents for sound medical practice and reimbursement.
