Job summary

Priority Dispatch Corp. is seeking an experienced Nurse Practitioner to oversee clinical care and treatment for patients and families in hospice settings.

You will consult with the patient’s attending physician, in consultation with the hospice physician, and provide clinical direction to the Interdisciplinary Group/Team to ensure quality care. You will collaborate with IDG/T to develop and monitor plans of care, educate patients and families, and provide on-call coverage while traveling to

Qualifications

• Current RN license in the state.
• APR.N license as Advanced Practice Nurse.
• Two years of APRN experience; hospice or home health preferred.
• Strong pharmacology, palliative care and end-of-life knowledge.
• Excellent interpersonal skills.

Responsibilities

• Collaborates with the IDG/T to assure appropriate evaluation and palliative treatment.
• Consults with attending physicians and hospice physician to develop patient plans.
• Educates patients and families about disease process and end-of-life care.
• Provides staff training as needed.
• Performs and documents home visits based on plan of care.
• Provides on-call services as assigned.
