The role collaborates with the Interdisciplinary Group, provides education, completes face-to-face encounters, and ensures quality care while maintaining documentation and communication with the medical team.

Qualifications

• Graduate from a Baccalaureate School of Nursing and NP-focused degree.
• Current state RN license and Advanced Practice Nurse license.
• Two years experience as an Advanced Practice Nurse, hospice or home health preferred.
• Knowledgeable about hospice care psychosocial, ethical, legal aspects.

Responsibilities

• Collaborates with the IDG/T to ensure appropriate evaluation and palliative treatment.
• Develops patient/family plan of care with IDG/T to ensure quality care.
• Educates patient/family about disease process, self-care, end-of-life care; trains staff as needed.
• Performs and documents home visits and face-to-face encounters per plan of care.
• Communicates with attending physicians and hospice team regarding palliation and care changes.
• Provides education to IDG/T on palliation and care issues as needed.
• Maintains timely and accurate documentation and complies with policies.
• Works with IDG/T to ensure patient/family needs are met and meets productivity standards.
