Job summary

Qualifications
- Master's degree with a minimum of two years hospice or palliative care experience as an RN or NP preferred.
- Holds current unencumbered RN and NP licenses in State of practice.
- Certifications in Hospice and Palliative Nursing or related areas preferred.
Responsibilities
- Deliver compassionate medical care for terminal illnesses under physician supervision.
- Collaborate with hospice inter-disciplinary team to assess eligibility and plan care.
- Coordinate patient care and ensure a holistic, patient-centric approach.
