Job summary

Heritage Hospice is seeking a skilled Nurse Practitioner to conduct Face-to-Face encounters for patients approaching benefit period recertification. You will assess prognosis, document disease trajectory, and collaborate with the IDG to inform recertification decisions.

Travel across the New Mexico service area is required, visiting patients in homes, assisted living facilities, and SNFs. A NM NP license and board certification are essential, with hospice experience preferred.

Qualifications

• Current and unrestricted NP license in New Mexico.
• Board-certified NP (FNP/AGNP or equivalent).
• Minimum 1 year clinical experience; hospice, palliative care, or geriatric experience preferred.
• Solid understanding of hospice eligibility criteria and disease trajectories.
• Strong communication, interpersonal, and clinical assessment skills.
• Reliable transportation and willingness to travel across territory.

Responsibilities

• Conduct Face-to-Face encounters for patients entering their 3rd benefit period and beyond.
• Assess and document terminal prognosis, disease progression, functional decline, and symptom burden.
• Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions.
• Provide clinical insight and education to staff regarding disease trajectories, eligibility criteria, and hospice best practices.
• Maintain timely, compliant documentation aligned with Medicare and state regulatory standards.
• Communicate findings effectively to physicians and clinical leadership.
• Travel within assigned New Mexico service area to visit patients in homes, assisted living facilities, SNFs, or other care settings.
