Job summary

Ohio Living seeks an Advanced Practitioner to provide patient care across home, facility, and clinic visits in Ohio. You will interpret symptoms, diagnose, prescribe, and coordinate with a multi-disciplinary team, while maintaining accurate EMR documentation.

Two years of advanced practice experience, preferably in geriatrics, and an active Ohio NP or PA license are required. Proficiency with Windows/Microsoft Office and strong communication help ensure compassionate, high-quality care.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner or Physician Assistant.
• Two years of experience as an advanced practitioner; geriatrics preferred.
• Experience in home health, hospice, physician office, or long-term care setting preferred.
• Proficiency with Windows, Microsoft Office, and EMR documentation.

Responsibilities

• Care and Relationship Management – interprets symptoms, obtains histories, performs exams, prescribes medications.
• Collaborates with physicians, PAs, nurses to form a high-performing medical team.
• Maintains comprehensive EMR documentation and ensures compliance with Medicare standards.
