Job summary

AccentCare in Washington, DC is seeking a part-time Social Worker / MSW I to support hospice patients at MedStar Washington Hospital Center. The role is on-site, with a schedule of M/W/F 8am-5pm and pay ranging from $30.00 to $46.00 per hour.

The ideal candidate will hold a Master's in Social Work, be licensed to practice, and have 1–2 years of healthcare social work experience, with preferred expertise in Medicare/Medicaid, discharge planning, and grief counseling.

Qualifications

• Master's degree in Social Work with licensure to practice in the relevant state.
• Medicare/Medicaid/Insurance specialty preferred.
• At least 1 year of post-graduate social work experience in healthcare; 2+ years preferred.
• Grief counseling experience preferred.
• Discharge planning experience preferred.

Responsibilities

• Assist core pre-hospice/hospice team in addressing social, spiritual, and emotional factors to plan care.
• Participate in interdisciplinary team and care plan development.
• Assess social, spiritual, and emotional needs of patient and caregiver capacity.
• Prepare patient for coping and educate family on advance directives.
• Utilize resources to support patient and family within illness limits.
• Support the bereavement program.
• Provide discharge planning for care level change or location.
