Job summary

Enhabit Home Health & Hospice is seeking a licensed MSW to provide intermittent social work services to patients, families, and care teams as part of the plan of care. You will collaborate with clinicians to support patient goals and coordinate resources.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Must be licensed or certified in the state of practice.
• Minimum of one year of social work experience in a health care setting.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other health care team members.
• Perform services in accordance with physician orders and the established plan of care (POC).
