Job summary

Madrona Hospice Care Inc. seeks a Medical Social Worker to lead our in-home social services team, mentor staff, and reinforce high-quality, compassionate care across the organization.

The role is per diem with a visit rate of $100-$120. You will oversee care coordination, recruit and supervise social workers, ensure compliant documentation, and collaborate with leadership on budgets and community partnerships.

Qualifications

• MSW from an accredited program.
• Current California LCSW license.
• Minimum 3 years of management experience (preferably home care).
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and ability to build trust.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination.
• Recruit, onboard, and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation of social services care.
• Partner with the Executive Director on the annual department budget and resource planning.
• Connect patients and referral sources to community resources.
• Plan in-service training and continuing education.
• Support quality improvement initiatives and patient service enhancements.
• Perform other duties as required for patient services.
