Job summary

Valir Hospice Care, LLC seeks an LPN Clinical Scheduler to coordinate daily interdisciplinary patient care schedules. You will align nursing staff and hospice team members to ensure timely, compliant care per physician orders and regulatory standards.

You will collaborate with the Director of Clinical Services and team to optimize staffing, support admissions, and adjust visit frequencies, while occasionally providing direct patient care within LPN scope as needed.

Qualifications

• Thorough understanding of hospice regulations and interdisciplinary care.
• Strong knowledge of visit frequency requirements.
• Excellent organizational and time management skills.
• Ability to prioritize multiple competing demands.
• Strong critical thinking and decision-making abilities.
• Excellent interpersonal and communication skills.
• Proficient with Microsoft Office and WellSky.
• Ability to work independently while collaborating with the clinical team.

Responsibilities

• Coordinate daily schedules for RNs, LPNs, Hospice Aides, Social Workers, Chaplains, Bereavement Coordinators, Volunteers and other staff.
• Balance staff caseloads by geography, acuity and resources.
• Adjust schedules for admissions, deaths, emergencies, and urgent needs.
• Monitor clinician productivity and scheduling efficiency.
