Job summary

Northern Arizona Hospice of Cottonwood, AZ, part of the LHC Group and aligned with the Optum family, seeks a skilled hospice nurse practitioner or clinical nurse specialist to lead patient care in home hospice settings.

You will provide direct patient visits, coordinate with the Hospice Medical Director and care team, and guide families in self-care while ensuring thorough documentation and quality outcomes. A robust benefits package supports professional growth.

Qualifications

• Active unrestricted nursing license in the state of residence.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
• Current national certification by a body such as ANCC, NBCHPN, or AANP.
• Current CPR certification.
• 5+ years of nursing experience.
• Valid driver's license and access to dependable vehicle or public transportation.

Responsibilities

• Make face-to-face visits to Hospice patients within prescribed time limits and report clinical findings to the Hospice Medical Director.
• Consult with the Hospice Medical Director and coordinate with primary physicians and other physicians as needed.
• Initiate and coordinate the plan of care; document problems, goals, interventions, and patient/family responses.
• Instruct and supervise patients and families in self-care techniques when appropriate.
• Maintain accurate clinical notes and report patient condition and needs to the Administrator and Hospice Medical Director.
