Job summary

Hospice of the Western Reserve (Reserve Care) is seeking a Licensed Practical Nurse in Ohio to deliver prescribed medical treatment and personal care in collaboration with the transdisciplinary team, coordinating the Plan of Care.

You will assess needs, provide skilled nursing, manage pain and symptoms, educate patients and families, and participate in care conferences, discharge planning and on-call rotations.

Qualifications

• Provide prescribed medical treatment and personal care services to terminally ill persons in collaboration with the transdisciplinary team.
• Coordinate the Plan of Care implementation using the Nursing Process.
• Assess physical, psychosocial, and spiritual needs of patient and family.
• Provide intermittent skilled nursing services under physician direction.
• Monitor and manage pain and symptoms and educate patient and family.
• Maintain and complete hospice care plans and reports.
• Participate in care conferences and bereavement follow-up.

Responsibilities
