Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking an experienced registered nurse to provide prescribed medical treatment and personal care to terminally ill patients in collaboration with a transdisciplinary team. You will coordinate the Plan of Care for each patient and assess physical, psychosocial, and spiritual needs in various settings.

The role requires analyzing pain and symptoms, educating patients and families, and acting as a liaison to physicians.

Qualifications

• Licensed in the State of Ohio.
• Experience preferred in hospice care, home health, oncology and/or pain and symptom management.
• Current CPR certification.
• Must have and maintain a valid driver’s license with a good driving record and auto insurance.
• Hospice and Palliative Care Certification desired. Will be required within first two years of hire.
• Equivalent education/experience may substitute for minimum qualifications.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Coordinate the Plan of Care with the transdisciplinary team.
• Assess physical, psychosocial, and spiritual needs of patient and family.
• Monitor and manage pain and symptoms and educate family.
• Serve as liaison to physicians and provide information for care decisions.
• Provide emotional support to patient and family.
• Participate in care conferences and discharge planning.
• Assist in on-call rotation and case reviews.
