Job summary

Reserve Care, formerly Hospice of the Western Reserve, delivers hospice, palliative, and grief support with a compassionate team to patients and families in Ohio. We seek a licensed RN to provide prescribed medical treatment and personal care in collaboration with the transdisciplinary team.

Qualifications

• Licensed in the State of Ohio.
• Experience preferred in hospice care, home health, oncology and/or pain and symptom management.
• Current CPR certification.
• Must have and maintain a valid driver’s license, good driving record and auto accident insurance.
• Hospice and Palliative Care Certification desired; to be obtained within two years of hire.
• Equivalent education/experience may substitute for minimum qualifications.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients in collaboration with the transdisciplinary team.
• Assess physical, psychosocial, and spiritual needs of the patient and family/caregivers.
• Provide intermittent skilled nursing services to the patient under the physician's direction.
• Monitor and evaluate pain and symptoms; inform and assist patient and family.
• Serve as liaison to the patient's physician; provide information based on patient needs.
• Provide emotional support to patient and family/caregiver.
• Refer patients/family to other services and follow up as needed.
• Provide health teaching to patient and family.
• Maintain and complete plans and reports related to hospice care.
• Participate in care conferences and bereavement follow-up.
• Participate in discharge planning with the team.
