Job summary

Affinity Health Management in Ohio is seeking an experienced Licensed Practical Nurse to join our hospice care team. The LPN will work with RNs to provide compassionate in-home and community-based nursing services, focusing on patient comfort and family education.

The role emphasizes performing routine assessments, administering prescribed medications, and coordinating with the interdisciplinary team to meet patient needs and ensure quality end-of-life care in the home setting.

Qualifications

• Graduate of an accredited school of nursing.
• LPN license in good standing to practice.
• Minimum 2 years nursing experience (last 3 years) or 1 year in public health/home/hospice preferred.
• Satisfactory references and background checks required.
• Excellent observation and communication skills; basic math and nursing skills.

Responsibilities

• Perform routine nursing assessments to determine hospice needs.
• Administer medications and treatments per physician plan.
• Re-evaluate patient status and update care plan as needed.
• Counsel patients and families on nursing care and self-care.
• Maintain accurate clinical notes and communications with physicians.
• Coordinate care with RN Case Manager and healthcare team.
• Participate in on-call duties and ensure required supplies.
• Promote safety and quality in patient care.
