Job summary

Ohio Living is seeking a Licensed Practical Nurse (LPN) to deliver quality home health and hospice care across Ohio. You will plan and implement patient care, coordinate with the care team, and ensure compliance with federal and state guidelines.

Responsibilities include assessing patient needs, managing care plans, and educating patients and families. A valid Ohio LPN license and driving ability are required; Medicare home health experience is preferred.

Qualifications

• Active LPN license in Ohio is required.
• One year of acute care experience is required.
• Experience in a Medicare-certified home health agency is preferred.
• Proficiency with Windows and Microsoft Office and EMR systems.

Responsibilities

• Provide ongoing nursing care in patients' homes and health care facilities, developing and implementing care plans.
• Follow nursing standards of practice and agency protocols for home health and hospice.
• Coordinate with other health care providers and educate patients and caregivers.
• Oversee and evaluate the patient’s plan of care and communicate changes.
