Job summary

Ohio Living is seeking an LPN Field Staff to plan, coordinate, and deliver quality home health and hospice care in Ohio. You will ensure compliance with state and federal guidelines while developing personalized care plans and educating patients and families.

The role requires current Ohio LPN licensure, at least one year in acute care, and preferred experience in Medicare-certified home health or medical-surgical settings. Driving and 24/7 on-call availability are required.

Qualifications

• Current unencumbered license for the state of Ohio to practice as an LPN.
• One year experience in acute care required.
• One year experience in medical-surgical nursing preferred.
• Experience in a Medicare-certified home health agency preferred.

Responsibilities

• Care management and service delivery in patients' homes and health care facilities.
• Develop and implement a comprehensive care plan addressing current needs.
• Identify when patients need additional services and notify the team.
• Follow nursing standards and agency protocols for home health and hospice.
• Educate patients and caregivers and communicate changes in patient condition.
• Oversee implementation of the patient’s plan of care and evaluate effectiveness.
• Prepare for IDG by reviewing care plans and reconciling medications.
