Qualifications

• Minimum of two years general nursing experience; hospice experience preferred.
• Experience with pain and symptom management interventions preferred.
• Working knowledge of hospice philosophy and principles of death and dying.
• Clearance by PA State Police criminal history check and PA Department of Public Welfare Childline.

Responsibilities

• Deliver direct nursing care to hospice patients.
• Coordinate interdisciplinary care and follow individualized care plans.
• Monitor patient condition, intervene, and educate patients and families.
• Address patient/family concerns promptly and effectively.
